From 16/03/20 Pathfields Medical Group under the guidance of Department Of Health have made the decision to triage all patients before entry to the surgery.  This temporary step is to protect both vulnerable individuals and the wider community from the risk of Covid-19. We have capacity to be able to help patients via: online consultations at www.pathfields.co.uk or telephone triage via for Pathfields Video consultations-where appropriate.

Urgent problems needing an examination will be booked by a GP.  All sick notes, urgent forms and letters can be arranged via eConsult .  Prescription can be ordered online or via your usual Pharmacy.

We hope to be back to normal as soon as possible, in the meantime hoping these measures will reduce the risks to yourselves, family & friends without compromising your healthcare.

VACCINATION TRANSPORT

Access Plymouth’s transport for the elderly and disabled to and from vaccination appointments. The initial advice for people travelling to and from vaccination appointments is to travel with their families or friends as this is the safest way to travel. However, this is not always possible and Access Plymouth has volunteer drivers and a minibus set up for socially distanced travel to support people to attend.

We envisage that that service will start in full on Thursday 8th January although we will be able to take bookings from Monday 4th January. Our telephone booking lines will be open from 9am until 2pm Monday to Friday.

What you need to know about us

· All drivers are DBS checked

· The drivers will be wearing PPE and everyone travelling with them should wear a mask unless they are exempt. (Passengers who cannot wear a mask will be asked to travel in the minibus to protect our drivers. This may reduce the availability of a lift as bus trips are limited).

· Passengers should travel alone if at all possible. However, one person can accompany them if necessary. (Please check for up to date government advice when travelling) All passengers must use the back seat. Front seats will not be available.

· The driver will take the passenger to the vaccination station, wait until the appointment is completed and then take them home.

· The cost of the car trip is £1.20 per mile which will be taken by phone before travel. The costs cover the drivers’ fuel. Trips to vaccinations on the minibus within the Plymouth area will cost £5 return. Distances over 4 miles may incur a small extra cost.

· There is a small hardship fund for people unable to meet the cost. However, we are a charity and these funds will be limited.

· Bookings can be made by medical professionals, families and patients by phoning 01752 600633 from Monday to Friday 10am until 2pm or by email at ccaccessplymouth@gmail.com Lifts will be available Monday to Friday from 9am until 4pm. (Although, if cars are available – there may be some flexibility on the times)

What we need from you

o Name and address of the patient

o Date of birth

o Contact phone number

o Time and destination of the appointment

o Any mobility aids that the patient will need to bring.
